TERMS OF SERVICE and INFORMED CONSENT


TERMS OF SERVICE

Please read these Terms of Service (“Terms”, “Terms of Service”) carefully before using any of the Seven.me (“Seven”) products or services (“Services”) operated by Seven.me Inc (“us”, “we”, or “our”).

Your access and use of the Service is conditioned on your acceptance of and compliance with these Terms. These Terms apply to all visitors, users and others who access or use the Services

INTRODUCTION TO SERVICE

Please take as much time as you need to read this form. You may want to discuss it with your family, friends, or your personal doctor. If you find any of the language difficult to understand, please ask questions using the contact information at the end of this document. KEY INFORMATION

The following is a short summary of Seven's current Services. More detailed information is listed later in this form.

  1. Utilizing and engaging with these Services is voluntary-it is your choice.
  2. You want to take part in this experience because you are concerned about your current health conditions.
  3. You understand that as part of participation in this program, biomarker data will be collected by an external lab at baseline and again at 12 weeks, and will be used to see the potential benefits coaching can have on your health.
  4. You understand that data derived from biomarker testing is to be considered preliminary only and does not constitute any kind of diagnosis.
  5. You agree to work closely with your healthcare provider while in the Seven coaching program for oversight of medication use/discontinuation, supervision of health metrics and review of lab work.
  6. Detailed information about the potential minimal risks of these Services can be found under the “Risk and Discomfort” section.

DETAILED INFORMATION

PROCEDURES

By using and engaging with these Services, you agree to:

  • Download Seven Coaching App and provide registration information including personal Information such as name, address, phone number, and date of birth
  • Complete coaching-based assessments and online coaching surveys;
  • Complete blood testing (at least two blood draws) at a designated location;
  • Complete blood testing within 10 days of session 1 completion;
  • Participate in cycles of 4 coaching sessions with Seven coaches via Seven app (via video calls). Please note that all coaching sessions will be recorded so we can capture all aspects of your interaction with these Services;
  • Engage with the Seven Coaching app regularly and provide feedback via surveys; and
  • Completing a final recorded video interview of the Services experience if asked of you by a member of the Seven team

RISKS/DISCOMFORTS

As blood collection is completed at an external laboratory, you will need to read and sign the external laboratory's terms and conditions agreement (which includes possible associated risks) prior to using their services. This agreement will be sent to you for your completion after your first Meet & Greet session with your coach via email.

Coaches do not provide professional diagnosis or treatment for physical or mental health interventions and should you require this during your coaching, you will be encouraged to seek out your primary care physician or specialist. As a participant, you understand that coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.

We also do not offer a 24/7 service or emergency service, and people should contact their own health provider or dial 911 in case of an emergency.

BENEFITS

If you agree to take part in these Services, there may or may not be a direct benefit to you based on the information learned from the different coaching sessions. Information learned from this experience will benefit Seven for future product iterations.

ALTERNATIVES

Use of these Services is not designed to diagnose, treat or present any disease. As an alternative you may choose to no longer utilize this provided service.

VOLUNTARY PARTICIPATION

Both Seven or the participant, without cause, have the right to terminate the relationship and/or use of these Services.

If either party chooses to discontinue the use of these Services, already collected data may not be removed from our database. No new information will be collected about you or from you by the product evaluation team without your permission.

A participant’s personal information including any identifiable information that has already been collected up to the time of your withdrawal will be kept confidential and used to guarantee the integrity of Seven Services and to satisfy any legal or regulatory requirements.

CONFIDENTIALITY AGREEMEN

As a participant using these Services, you understand that you will have access to confidential information about Seven’s program, its execution, questionnaires and other product details. By signing this policy, you understand that all information provided using these Services is completely confidential. You shall not divulge, publish, or otherwise make known to unauthorized persons or to the public any information obtained in the course of your access to Seven Services.
